All your records are kept in strictest confidence in line with General Data Protection Regulation 2018 (GDPR).
Confidential case notes are kept in a lockable folder and any data kept on electronic devices such as a laptop are password protected.
Information is only ever shared with other individuals or organisations with your consent.
I destroy information no longer needed.
I only record information relevant to your treatment(s).
You have the right to access a copy of information kept about you. This is called a "Subject Access Request" & I have 1 month to provide a copy of your records to you, free of charge, after you have submitted a Subject Access Request.
You have the "right to rectification" which means you can ask me to update anything that is factually incorrect about you.
You have the "right to erasure" & the "right to be forgotten". This means you can request for all your records to be deleted.
You have the right to ask me to stop sending you any promotional material at any point. I rarely send this kind of material.
You have the right to complain to the Information Commissioner's Office (ICO) if you have any concerns about privacy. I trust clients would be able to contact me first to resolve any issues, but should you need to make a complaint to the ICO, you can contact them at: https://ico.org.uk/
